If you choose to use any of my services such as a consultation or online plan I will collect information about your health via verbal discussion and examination and document it to enable me to provide a health care service. Paper documents will be stored for the duration of my insurance after which time they will be shredded. Your information will be kept a locked filling cabinet, only myself or an administrator has access to these files. Any documentation collected digitally or by email is password protected. All your data is considered confidential and will not be shared with a 3rd party unless I have your permission. You are entitled to a copy of your health records which might incur an administration fee. If you would like a copy of, or infomation concerning the data held on you, please contact me directly.
